  • Cancellation of convention fees should be made in writing not later than July 15, 2016 to be eligible for refund, subject to 30% administration charge. Original copy of the receipt must be surrendered to be eligible for the refund which will be made only after the convention. NO REFUND will be given for cancellation made after the said date. However, a substitute delegate will be accepted.

  • Each delegate is required to fill-up this form to update the membership roster.
